When faced with adverse circumstances, there may be a tendency for individuals, agencies, and governments to search for a target to assign blame. Our focus will be on the novel coronavirus outbreak, where racial groups, political parties, countries, and minorities have been blamed for spreading, producing or creating the virus. Blame—here defined as attributing causality, responsibility, intent, or foresight to someone/something for a fault or wrong—has already begun to damage modern society and medical practice in the context of the COVID-19 (...) outbreak. Evidence from past and current pandemics suggest that this tendency to seek blame affects international relations, promotes unwarranted devaluation of health professionals, and prompts a spike of racism and discrimination. By drawing on social and cognitive psychology theories, we provide a framework that helps to understand the effect of blame in pandemics, when people blame, whom they blame, and how blame detrimentally affects the COVID-19 response. Ultimately, we provide a path to inform health messaging to reduce blaming tendencies, based on social psychological principles for health communication. (shrink)

How do people understand questions about cause and prevent? Some theories propose that people affirm that A causes B if A's occurrence makes a difference to B's occurrence in one way or another. Other theories propose that A causes B if some quantity or symbol gets passed in some way from A to B. The aim of our studies is to compare these theories' ability to explain judgements of causation and prevention. We describe six experiments that compare judgements for causal (...) paths that involve a mechanism, i.e. a continuous process of transmission or exchange from cause to effect, against paths that involve no mechanism yet a change in the cause nevertheless brings about a change in the effect. Our results show that people prefer to attribute cause when a mechanism links cause to effect. In contrast, prevention is sensitive both to the presence of an interruption to a causal mechanism and to a change in the outcome in the absence of a mechanism. In this sense, ‘prevent’ means something different than ‘cause not'. We discuss the implications of our results for existing theories of causation. (shrink)

Metabolic pathways underlying brain function remain largely unexplored during neurodevelopment, predominantly due to the lack of feasible techniques for use with awake infants. Broadband near-infrared spectroscopy provides the opportunity to explore the relationship between cerebral energy metabolism and blood oxygenation/haemodynamics through the measurement of changes in the oxidation state of mitochondrial respiratory chain enzyme cytochrome-c-oxidase alongside haemodynamic changes. We used a bNIRS system to measure ΔoxCCO and haemodynamics during functional activation in a group of 42 typically developing infants aged between (...) 4 and 7 months. bNIRS measurements were made over the right hemisphere over temporal, parietal and central cortical regions, in response to social and non-social visual and auditory stimuli. Both ΔoxCCO and Δ[HbO2] displayed larger activation for the social condition in comparison to the non-social condition. Integration of haemodynamic and metabolic signals revealed networks of stimulus-selective cortical regions that were not apparent from analysis of the individual bNIRS signals. These results provide the first spatially resolved measures of cerebral metabolic activity alongside haemodynamics during functional activation in infants. Measuring synchronised changes in metabolism and haemodynamics have the potential for uncovering the development of cortical specialisation in early infancy. (shrink)

State Medical Boards (SMBs) can take severe disciplinary actions (e.g., license revocation or suspension) against physicians who commit egregious wrongdoing in order to protect the public. However, there is noteworthy variability in the extent to which SMBs impose severe disciplinary action. In this manuscript, we present and synthesize a subset of 11 recommendations based on findings from our team’s larger consensus-building project that identified a list of 56 policies and legal provisions SMBs can use to better protect patients from egregious (...) wrongdoing by physicians. We report the identification of a recurrent, 520-kb 16p12.1 microdeletion associated with childhood developmental delay. The microdeletion was detected in 20 of 11,873 cases compared with 2 of 8,540 controls and replicated in a second series of 22 of 9,254 cases compared with 6 of 6,299 controls. Most deletions were inherited, with carrier parents likely to manifest neuropsychiatric phenotypes compared to non-carrier parents. Probands were more likely to carry an additional large copy-number variant when compared to matched controls. The clinical (...) features of individuals with two mutations were distinct from and/or more severe than those of individuals carrying only the co-occurring mutation. Our data support a two-hit model in which the 16p12.1 microdeletion both predisposes to neuropsychiatric phenotypes as a single event and exacerbates neurodevelopmental phenotypes in association with other large deletions or duplications. Analysis of other microdeletions with variable expressivity indicates that this two-hit model might be more generally applicable to neuropsychiatric disease. © 2010 Nature America, Inc. C.C.E. Schmid’s doctrine of intelligible fatalism was immensely influential in the immediate reception of Kant’s philosophy. Existing treatments of this doctrine, largely neglected by modern scholarship, echo uncharitable interpretations espoused by Schmid’s contemporaries. I demonstrate that Schmid’s intelligible fatalism is more coherent and philosophically robust than hitherto recognized. I argue for a novel interpretation of Schmid’s account of rational agency, showing that intelligible fatalism is compatible with his conceptions of freedom, obligation, and imputation. Specifically, I argue that the role of (...) consciousness in this account carves out conceptual space for a distinction between the theoretical and the practical that is sufficient to render intelligible fatalism consistent with these conceptions. There are two general approaches to characterising biological functions. One originates with Cummins. According to this approach, the function of a part of a system is just its causal contribution to some specified activity of the system. Call this the ‘C-function’ concept. The other approach ties the function of a trait to some aspect of its evolutionary significance. Call this the ‘E-function’ concept. According to the latter view, a trait's function is determined by the forces of natural selection. The C-function (...) and E-function concepts are clearly quite different, but there is an important relation between them which heretofore has gone unnoticed. The purpose of this paper is to outline that relation.This is not the first paper to discuss the relation of C-function and E-function. Previous attempts all follow either one of two strategies. The first proposes that the two concepts are ‘unified.’ The other proposes that they are radically distinct and apply to wholly different fields within biology. (shrink)

As such, the text is usually dated to the mid first centuryc.e.This view is so established that certain scholars have suggested it is ‘little short of perverse not to accept the general consensus and read theSatyricaas a Neronian text of the mid-60sad’. In recent years, however, there has been a groundswell of support for re-evaluating this long-held position. Laird, after comparing the ‘form and content’ of (...) the text to the Greek novel, came to the ‘unattractive’ conclusion that the text may be second century. Similarly, in two recent pieces inCQ, Roth argues that the manumission scene in theCenaestablishes a newterminus post quemfor the text; she suggests that the freedoms granted by Trimalchio closely parallel—and parody—descriptions of awardingciuitasfound in the letters of Pliny the Younger. Indeed, the three slaves manumitted in the novel are associated with a boar (Sat.40.3–41.4), Dionysus (Sat.41.6–7) and a falling star (Sat. 54.1–5); likewise, the three slaves that are the subject of Pliny's letter are C. Valerius Aper (boar), C. Valerius Dionysius (god of wine) and C. Valerius Astraeus (stars). Roth's argument suggests that the author of theSatyricawas not Nero's contemporary but a member of Pliny's intellectual circle, offering strong circumstantial evidence that troubles the accepted tradition on the work's authorship and date. In this paper, I outline the historical context of C.C.E. Schmid's doctrine of intelligible fatalism. By doing so, I show the development of this influential doctrine and sketch Schmid's apparent revision of it in light of contemporary criticisms.

This article improves two existing theorems of interest to neologicist philosophers of mathematics. The first is a classification theorem due to Fine for equivalence relations between concepts definable in a well-behaved second-order logic. The improved theorem states that if an equivalence relation E is defined without nonlogical vocabulary, then the bicardinal slice of any equivalence class—those equinumerous elements of the equivalence class with equinumerous complements—can have one of only three profiles. The improvements to Fine’s theorem allow for an analysis of (...) the well-behaved models had by an abstraction principle, and this in turn leads to an improvement of Walsh and Ebels-Duggan’s relative categoricity theorem. In a recent unit of study in an undergraduate Health Sciences pathway course, we identified a set of essays which exhibited similarity of content but demonstrated the use of bizarre and unidiomatic language. One of the distinct features of the essays was the inclusion of unusual synonyms in place of expected standard medical terminology.We suspected the use of online paraphrasing tools, but were also interested in investigating the possibility of the use of online language translation tools. In order to test (...) the outputs of these tools, we used as a seed document a corpus of text which had been provided to the students as prompt for the essay. This document was put through six free online paraphrasing tools and six separate iterative language translations through the online Google Translate™ tool.The results demonstrated that free online paraphrasing tools did not identify medical terminology as standardised or accepted nomenclature and substituted synonyms, whereas Google Translate™ largely preserved medical terminology.We believe that textual indicators such as the absence of standard discipline-based terminology may be of assistance in the identification of machine paraphrased text. We begin this article by delineating the explanatory gaps left by prevailing gene‐focused approaches in our understanding of phenotype determination, inheritance, and the origin of novel traits. We aim not to diminish the value of these approaches but to highlight where their implementation, despite best efforts, has encountered persistent limitations. We then discuss how each of these explanatory gaps can be addressed by expanding research foci to take into accountbiological agency—the capacity of living systems at various levels to participate in (...) their own development, maintenance, and function by regulating their structures and activities in response to conditions they encounter. Here we aim to define formally what agency and agents are and—just as importantly—what they are not, emphasizing that agency is an empirical property connoting neither intention nor consciousness. Lastly, we discuss how incorporating agency helps to bridge explanatory gaps left by conventional approaches, highlight scientific fields in which implicit agency approaches are already proving valuable, and assess the opportunities and challenges of more systematically incorporating biological agency into research programs. In the UK, current policies and services for people with mental disorders, including those with intellectual disabilities (ID), presume that these men and women can, do, and should, make decisions for themselves. The new Mental Capacity Act (England and Wales) 2005 (MCA) sets this presumption into statute, and codifies how decisions relating to health and welfare should be made for those adults judged unable to make one or more such decisions autonomously. The MCA uses a procedural checklist to guide this (...) process of substitute decision-making. The personal experiences of providing direct support to seven men and women with ID living in residential care, however, showed that substitute decision-making took two forms, depending on the type of decision to be made. The first process, ‘strategic substitute decision-making’, paralleled the MCA’s legal and ethical framework, whilst the second process, ‘relational substitute decision-making’, was markedly different from these statutory procedures. In this setting, ‘relational substitute decision-making’ underpinned everyday personal and social interventions connected with residents’ daily living, and was situated within a framework of interpersonal and interdependent care relationships. The implications of these findings for residential services and the implementation of the MCA are discussed. (shrink)

Surgeons have an ethical obligation to ensure all patients, regardless of their personal characteristics, receive the same quality of care. Established surgeons also have an obligation to ensure equal treatment for their peers and for those who would like to join the field. The commitment to ethical hiring and working standards entails making certain all individuals have the same opportunities free from discriminatory practices. The world of business has long realized the positive implications of having a diverse and inclusive workforce. (...) Studies have shown a positive correlation between increased racial and gender diversity and company performance. Diversity can lead to an increase in knowledge by sharing ideas with individuals who come from different life experiences and breakdown cultural barriers. We have already begun to see the impact of ignoring the importance of diversity within healthcare, including surgery. Healthcare disparities are defined by the NIH as “differences in the incidence, prevalence, mortality, and burden of diseases and other adverse health conditions that exist among specific population groups.” Healthcare disparities persist despite decades acknowledging their existence, and they are pernicious and prevalent in surgery. One proposed method to help reduce or eliminate healthcare disparities is to increase the diversity of the workforce treating the patients, which would in turn reduce potential bias the patients face in the healthcare setting. Implicit bias can play a key role in interactions with patients and colleagues and needs to be explored on an individual basis. Cultural competence training can also provide surgeons the ability to effectively communicate with not only a diverse population of patients but also a diverse surgical workforce. All surgeons regardless of their role can have a positive impact on diversity as it relates to patient care and the surgical workforce. (shrink)

Activists’ investigations of animal cruelty expose the public to suffering that they may otherwise be unaware of, via an increasingly broad-ranging media. This may result in ethical dilemmas and a wide range of emotions and reactions. Our hypothesis was that media broadcasts of cruelty to cattle in Indonesian abattoirs would result in an emotional response by the public that would drive their actions towards live animal export. A survey of the public in Australia was undertaken to investigate their reactions and (...) responses to. The most common immediate reaction was feeling pity for the cattle. Women were more likely than men to feel sad or angry. Most people discussed the media coverage with others afterwards but fewer than 10 % contacted politicians or wrote to newspapers. We conclude that the public were emotionally affected by the media coverage of cruelty to cattle but that this did not translate into significant behavioral change. We recommend that future broadcasts of animal cruelty should advise the public of contact details for counseling and that mental health support contacts, and information should be included on the websites of animal advocacy groups to acknowledge the disturbing effect animal cruelty exposes can have on the public. Every healthcare organisation enacts a multitude of policies, but there has been no discussion as to what procedural and substantive requirements a policy writing process should meet in order to achieve good outcomes and to possess sufficient authority for those who are asked to follow it.Using, as an example, the controversy about patient’s refusal of blood transfusions, I argue that a hospital wide policy is preferable to individual decision making, because it ensures autonomy, quality, fairness, and efficiency.Policy writing for morally (...) controversial medical practices needs additional justification compared to policies on standard medical practices and secures legitimate authority for HCO members by meeting five requirements: all parties directed by the policy are represented; the deliberative process encompasses all of the HCO’s obligations; the rationales for the policy are made available; there is a mechanism for criticising, and for evaluating the policy. A frequently recounted episode in early modern medicine concerns the physician Helvetius's introduction of ipecacuanha to French medical practice after curing Louis XIV's son of dysentery using this medicinal drug. To this day, the Helvetius story remains riven with contradictions, obscurity, and confusion, even down to the nature of the drug involved. This article, challenging histories of “information” as homogeneous and neutral, explores how Helvetius's reputation as a physician and pharmaceutical entrepreneur was crafted through print and correspondence. Rather than seeking (...) to establish a definitive account of “what actually happened,” it addresses the ways in which different media shaped and mediated the politics of knowledge surrounding Helvetius and his drug. Considerations of intellectual and commercial property inflected medical knowledge in different ways, producing distinct strategies of publicity. While Helvetius capitalized on courtly connections to promote himself and his drug, rivals eyed the Republic of Letters as an alternate route for establishing natural knowledge-claims. Yet the arch-newsmonger of the Republic of Letters, Leibniz, elected to preserve the connection between Helvetius and ipecacuanha in print. I argue that Leibniz's actions stemmed from a view of the public domain that downplayed discovery and emphasized disclosure of medical secrets in the public interest. (shrink)

: Recently developed projective models of consciousness and its contents challenge received schemas in which all contents of consciousness are held to be well contained in the skull. Working our way into this from several angles, it becomes evident that there are inconsistencies in how we frame classes of mental contents which are arguably equivalent in being. Particular examples of imagery, of dancing and of words, are brought forward to highlight the clash in our apprehensive assumptions, focusing on possible cognitive (...) as well as psychological costs of such inconsistency. A coherent way to blend the container and projector schemas is pointed out; yet such a blend does not support the standard claim for any kind of purely inner voice, contained but not projected. Conscious reflection may in all instances depend on projection, with reflection in imagery - visual, audible, even tactile and otherwise felt - placed just the far side of what can schematically be grasped as the sensory horizon where, in the space-time just beyond here and now, our imaginative expectations backstop recognition of here-now present things. Resumo: Os modelos projetivos de consciência e de seus conteúdos, recentemente desenvolvidos, desafiam os esquemas tradicionais, nos quais todos os conteúdos da consciência são mantidos bem contidos dentro do crânio. Trabalhando desta forma, a partir de vários ângulos, torna-se evidente que existem inconsistências na forma como emolduramos classes de conteúdos mentais que são possivelmente equivalentes em seu ser. Exemplos particulares de imagens, de dança e de palavras são apresentados para ressaltar o choque em nossas suposições apreensivas, focalizando possíveis custos cognitivos e psicológicos de tal inconsistência. Uma maneira coerente de misturar os esquemas do “continente” e do “projetor” é apontada; no entanto, tal combinação não suporta as exigências gerais, encontradas na literatura, para qualquer tipo de fala puramente interna, que esteja contida, mas não projetada. Involuntary autobiographical memories are typically discussed in the context of negative memories such as trauma ‘flashbacks’. However, IAMs occur frequently in everyday life and are predominantly positive. In spite of this, surprisingly little is known about how such positive IAMs arise. The trauma film paradigm is often used to generate negative IAMs. Recently an equivalent positive film was developed inducing positive IAMs . The current study is the first to investigate which variables would best predict the frequency of positive IAMs. (...) Higher levels of positive mood change to the film were significantly associated with the number of positive IAMs recorded in the subsequent week. Results demonstrate the importance of positive emotional reaction at the time of an event for subsequent positive IAMs. (shrink)
